Now let’s get you ready for your first visit. You can use OptiMantra phone app to pre-register for your first visit. This will save you time on the day of your appointment. You can pre-register online here. Click Here to do it online now:

On the App, you will be able to:

  1. Register as a patient
  2. Enter your insurance details
  3. Enter your medical history
  4. Enter your medications and allergies
  5. Send messages to our staff if you have any problems

If you can’t do it now, you can still show up 30 minutes early and ask the front staff to give you a Tablet or Paper Forms to register in the waiting room on the day of your visit.
